Talked a bit to Dick Hardt at yapc, and a quick update: he seems to think that if MacPerl already has the MULTIPLICITY work done, then as soon as MacPerl is ported to perl 5.6 (threads and all), then the in-process fork might basically just work. Would be nice. A beta might be available in July, probably by TPC 3.0.
