The only thing I can think of is that you should throw the file out and try again. Since it's Compress::Zlib::isaFilename that's complaining, the gzip might be corrupted, which isn't that unusual. CPAN will download a new one. CPAN 1.47 is out, as CPAN itself will tell you. According to the change log, since Chris' patches are in, we should now be able to "install CPAN", "reload cpan" and it should work. I'm going to try it later.
