Just to be clear, 'index' can be larger than 1. Very few commercial CDs use index marks (other than as pre-track pause indicators), and even fewer players can access them. My copy of Glenn Gould doing the Goldberg Variations is (if I recall correctly) all one track, with an index point at each variation. This is silly, and may have been fixed in later pressings. Also the title track on the Grateful Dead's "Terrapin Station" has index marks indicating the sections of the song. So 0 means pause, and nonzero means music.
